Ready to enroll in Devoted Health for 2027?
1. Email us at medicareteam@ddibenefits.com. Say “I’d like to enroll in Devoted” and include your name and phone number.
2. Sign your Scope of Appointment. This is a short consent form Medicare requires before we can help you. We’ll email it to you for electronic signature. (Unless we already have one on file)
3. We prepare your application. There are no forms for you to fill out.
4. Starting October 15, check your email. Review your application and sign it electronically. That’s it!
Your new coverage begins January 1, 2027. Enroll by December 31 so there’s no gap in coverage.

Preparing for a Workshop
Find out if Devoted is a good fit for you.
Take a moment to visit Devoted Health’s website and check to see if your current providers are in network and prescriptions are covered. Be sure to have names and dosages handy when you search!
What to Bring to the Workshop
- Your red, white and blue Medicare card or Medicare number
- Your doctor and prescription list with dosages
Save time: sign your consent form ahead of time! Medicare requires a short consent form, called a Scope of Appointment, before we talk about plans. Reply to this email if you are attending a workshop and we’ll send you a form for electronic signature. If you would rather wait, you can sign it at the workshop.
What to Expect at the Workshop
1. A friendly greeter will welcome you and check you in.
2. A licensed agent will give you a short overview of the plan and explain how the application works.
3. If you’d like to move forward with the plan, you will fill out your own application at a table, and an agent will guide you through each section.
4. We will double-check your application before you leave so nothing is missed.
5. You leave having done your part to enroll in your 2027 Medicare Plan!
