As we just experienced during the renewal season, employer-sponsored health insurance continues to face significant upward pressure both in Oregon and across the country. This newsletter provides a snapshot of the current landscape and strategies to help manage your benefits budget in preparation for 2027.

The 2026 Premium Landscape

National Trends

Highest Increase in 15 Years: National employer health benefit costs are projected to rise between 6.5% and 10% in 2026.

Key Drivers: Rising medical inflation, public programs (Medicare and Medicaid) cost shift to private commercial market, increased utilization of services, and the high cost of specialty drugs—particularly GLP-1 medications for weight loss—are the primary factors.

Total Cost per Employee: On average, total health benefit costs are expected to surpass $18,500 per employee.

Oregon Specifics

Small Group Market: Oregon regulators approved 2026 rates for small businesses (50 or fewer employees), with a weighted average increase of 11.5%.

Individual Market: For those not covered by a group plan, average rates in the individual market are rose by 9.7%.

Carrier Variations: Approved rate changes vary significantly by provider, ranging from 3.9% (PacificSource) to 21.5% (Providence).

Strategies to Improve Costs

With nearly 60% of employers planning cost-cutting changes for 2026, here are several actionable ideas to mitigate premium hikes:

Optimize Plan Design

High-Deductible Health Plans (HDHPs): Pair these with Health Savings Accounts (HSAs) to lower premiums while offering tax-advantaged savings for employees.

Tiered Networks: Incentivize employees to use “Centers of Excellence” or high-performance networks that offer better value and lower copays.

Leverage Alternative Funding Models

Level-Funding: For mid-sized groups, level-funded plans can offer the predictability of a fully insured plan with the potential for surplus refunds if claims are low.

Association Health Plans: Some small businesses can gain “large group” benefits by joining AHP’s, leading to lower rates.

Active Pharmacy Management

Biosimilar Adoption: Encourage the use of biosimilars for expensive biologic drugs to capture significant savings.

GLP-1 Strategy: Implement strict utilization criteria or pair coverage with lifestyle coaching to manage the high cost of weight-loss medications.

Invest in Prevention and Primary Care

Advanced Primary Care: Invest in models that emphasize chronic disease prevention, which can reduce expensive emergency room visits and hospitalizations.

Telehealth Expansion: Use virtual-first options for mental health and urgent care to provide a lower-cost alternative to in-person visits.
